Assertive Community Treatment (ACT)
ACT is an intensive, community-based behavioral health service designed for individuals with serious mental illness who need a higher level of support than traditional outpatient care. ACT uses a multidisciplinary team approach to provide comprehensive, person-centered treatment, rehabilitation, and support services wherever individuals live, work, and spend their time. The goal is to promote recovery, reduce psychiatric hospitalizations, improve housing stability, and help individuals live successfully in the community.
The Recovery and Wellness Center
The Recovery and Wellness Center helps people overcome substance use challenges while managing mental health conditions. We offer individualized treatment, group counseling, psychiatric care, peer support, and recovery coaching in a welcoming and supportive environment. Whether someone needs weekly support or a more intensive treatment program, our team works alongside each person to build the skills and confidence needed for long-term recovery and wellness.
Our center serves individuals with co-occurring substance use and mental health disorders who need integrated treatment in a community setting. Through ASAM Level 1 Outpatient and Level 2 Intensive Outpatient Services, the program offers evidence-based treatment, psychiatric support, medication-assisted treatment coordination, recovery support services, and care navigation designed to reduce emergency department utilization, hospital readmissions, homelessness, and justice system involvement while promoting sustained recovery and community integration.
The Community Support Program
The Community Support Program serves individuals with serious mental illness who need ongoing rehabilitation and recovery support but do not require the intensity of ACT services. Services are delivered in the community and focus on skill development, care coordination, housing stability, employment support, benefits navigation, wellness management, and linkage to medical, behavioral health, and social services. The program helps individuals maintain recovery, improve functioning, and avoid unnecessary hospitalizations and crisis service utilization.
Outpatient Behavioral Health Clinic
The Outpatient Clinic is often the first stop for adults seeking support at Community Connections. The clinic offers therapy and low-intensity services for those dealing with life stress, mental health symptoms, or healing from trauma.
Our interdisciplinary team works with each person to create a treatment plan that fits their unique needs. We take time to make sure consumers are matched with the right services and level of care, using a patient-centered approach every step of the way.
The Psychiatric Clinic at Community Connections provides comprehensive medication management services for all consumers who are in need of psychiatric care. Board-certified psychiatrists and experienced nurse practitioners are available Monday through Friday to offer individualized support and ensure safe, effective treatment.
On-Site Pharmacy: Genoa
Genoa is a full-service pharmacy located right in the main waiting room at Community Connections. Operated by Genoa, a QoL Healthcare Company with over 15 years of experience serving the behavioral health and addiction treatment community, this on-site pharmacy makes it easier for consumers to access the medications they need.
Genoa works closely with treatment teams and fills prescriptions from psychiatrists, primary care doctors, and other specialists. By coordinating care directly with providers, Genoa helps improve safety, convenience, and overall support for our consumers health and recovery.
On-Call Crisis Services
When crises happen after hours, our Crisis Service Team is here to help. Available after 5 PM on weekdays, and all day on weekends and holidays, this team supports all Community Connections consumers during urgent situations.
Our trained mental health professionals respond quickly to stabilize psychiatric crises, prevent harm, and reduce the need for emergency rooms or 911/988 calls. We focus on keeping individuals safe while helping to regain stability.
