Assertive Community Treatment (ACT)
The Assertive Community Treatment (ACT) program continues to deliver intensive, person-centered care to some of our community’s most vulnerable residents. Over the past fiscal year, the ACT team provided over 45,000 services to 500 consumers, ensuring continuity of care, crisis intervention, and support for independent living.
Strong Housing and Health Outcomes
Community Connections served 1,040 participants—700 single adults, 340 families, and 600 youth. Nearly 90% had insurance, 80% were stably housed, and 42 newly housed since October 2024. Hospitalizations declined, with an average 12-day follow-up, underscoring strong housing and health results.
Housing Support Care Management Progress
In May 2025, Housing Support Care Management (HSCM) supported 50 individuals in unstable housing, completing 125 outreaches and 13 hotline responses. Sixteen had co-occurring disorders and 27 physical health conditions. Despite challenges, there were no unexpected hospitalizations, overdoses, or fatalities.
Outreach and Community Response
Community Connections CSON Outreach team serves a large number of unhoused residents on a daily basis. During the surge the team has actively engaged and served approximately 75 residents a day. On certain days of the week the team has engaged and served up to 111, providing hope, dignity, respect, and critically needed resources to individuals served.
1,600 people found healing, stability, and renewed hope through over 4,200 clinical sessions delivered by our dedicated professionals across our programs.
First Annual Community Partnership Convening
Community Connections proudly hosted its First Annual Partnership Convening, bringing together leaders, partners, and community members for a day of meaningful dialogue and celebration. The event opened with welcoming remarks from CEO Damien Cabezas, setting the tone for a day centered on collaboration, equity, and community impact. Attendees then heard a powerful keynote address from DC Councilmember Christina Henderson, followed by an engaging fireside chat with distinguished panelists Ed Barry, Dr. Ayanna Bennett, Jeremy Attermann, and Karen Dale. Together, they explored urgent issues in health, equity, behavioral health, and cross-sector partnership. The convening concluded with the presentation of three honors and one surprise honoree: the Person of the Year Award presented to Dr. Maxine Harris, the Consumer of the Year Award for Marcia Lee Hughes, the Lifetime Achievement Award presented to Joseph Cullinan, and the Bold & Courageous Partners of the Year Award recognizing DC Central Kitchen and Capital One Bank. This inaugural gathering marked an inspiring step forward in strengthening partnerships across the District.
